Hi everyone! My name is Jillilan Leon, and I am a student at Sage Creek High School, currently working on a Passion Project called Memory Maker to raise awareness and support for Alzheimer's San Diego. As a part of this effort, I am creating a donation page and a small tribute website to honor those who have been affected by Alzheimer's disease. If you have a friend, family member, or loved one that have lived or are currently impacted by Alzheimer's, I would be incredibly grateful if you would like to share their story. 

Using the Google Form linked below, you can upload a photo of the person you want to honor, as well as a description about them (who they were, how you want them to be remembered, how they inspired you, etc). My goal is to create a collection of stories and photos that celebrates and remembers these individuals, while also raising awareness about Alzheimer's disease.
